Breast reconstruction with tissue expanders is a common method used to rebuild the shape of the breast after a mastectomy (surgical removal of the breast). This procedure involves a two-step process to gradually stretch the skin and muscle to make room for a permanent breast implant. Here’s a detailed explanation of the procedure:

Step 1: Placement of Tissue Expanders

  1. Initial Surgery: After the mastectomy, a tissue expander (a balloon-like device) is placed under the skin and possibly under the chest muscle.  This typically adds 30-60 minutes of operative time following your mastectomy.
  2. Positioning: The expander is usually positioned just above the pectoral muscle to create a natural-looking breast shape.
  3. Inflation Port: The expander has an integrated small valve or port which can be easily accessed through the skin

 

Step 2: Expansion Process

  1. Gradual Inflation: Beginning two to three weeks after surgery typically, the expander is gradually filled with saline (saltwater solution) through the valve. This process is done in the surgeon’s office with typically little to no discomfort.
  2. Expansion Sessions: Depending on your goals for breast size, expansion visits will be every 1-2 weeks for 1-2 months. 
  3. Monitoring Progress: The surgeon monitors the progress to ensure the skin and muscle are stretching adequately and without complications.

 

Step 3: Replacement with Permanent Implant

  1. Final Surgery: Once the desired size and shape are achieved, a second surgery is performed to remove the tissue expander and replace it with a permanent breast implant (silicone or saline).  This typically happens three months after your final expansion.
  2. Customization: The surgeon may use different techniques to create a natural look, including adjusting the implant position and perform fat grafting to improve the shape of the breast.

 

Recovery and Follow-up

  1. Recovery Time: Recovery from the initial surgery typically takes a few weeks. Patients might experience discomfort, swelling, and bruising, which gradually subside.
  2. Activity Restrictions: Patients are usually advised to avoid strenuous activities and heavy lifting during the recovery period which may last 6-8 weeks.
  3. Monitoring and Care: Regular follow-up appointments are necessary to monitor healing and the progress of tissue expansion.

Dr Jablonka is a phenomenal surgeon and he truly cares for his patients. I had a breast reconstruction with DIEP flap post prophylactic double mastectomy from 12 years prior. He removed the old implants using my own fat and placing them above the pectoral muscle not under like before. The look and the feel is so natural. I don’t have that constant tightness and hardness in my breasts. I feel and look normal again. He has been very kind throughout the process. He explained everything patiently every time.
