Simultaneous Implant Exchange with Fat Grafting (SIEF) is a breast reconstruction technique often used for women who are not satisfied with the results of a previous breast implant reconstruction or those experiencing complications related to their breast implants, such as capsular contracture (a condition where scar tissue forms around the implant, causing discomfort or deformity). SIEF combines the removal of existing implants with the transfer of the patient’s own fat to reconstruct the breasts in a single surgery.

 

Key Aspects of Simultaneous Implant Exchange with Fat Grafting:

  1. Implant Removal:
    • The first step in SIEF involves removing the existing breast implants. This is often done when the implants have complications, such as rupture, capsular contracture, or simply when the patient desires a more natural reconstruction.
    • The surgeon also removes the surrounding scar tissue or capsule, especially if it has thickened or hardened (capsular contracture).
  2. Fat Harvesting:
    • After the implants are removed, the surgeon harvests fat from other areas of the patient’s body, such as the abdomen, thighs, or flanks, using liposuction.
    • The harvested fat is then processed and purified to separate healthy fat cells from other fluids and debris.
  3. Fat Grafting (Lipofilling):

    • Donor Site Considerations: The donor areas from which fat is harvested will undergo liposuction, which carries its own risks and recovery considerations.
  7. Recovery:
    • Recovery from SIEF involves healing both from the implant removal and the fat grafting procedure. The recovery time may vary depending on the extent of the surgery and the individual patient’s healing process.
    • Patients typically experience some bruising and swelling in both the donor sites and the breasts, which gradually subsides over a few weeks.

Dr Jablonka is a phenomenal surgeon and he truly cares for his patients. I had a breast reconstruction with DIEP flap post prophylactic double mastectomy from 12 years prior.
