Oncoplastic Reconstruction after Lumpectomy

Oncoplastic reconstruction is a surgical technique used in breast cancer treatment that combines oncologic surgery (to remove cancer) with plastic surgery techniques to optimize the cosmetic outcome of the breast. It is typically performed after a lumpectomy, where part of the breast is removed to excise the tumor. The goal of oncoplastic reconstruction is to preserve the natural appearance of the breast or improve it, even after a significant portion has been removed due to cancer.

Key Components of Oncoplastic Reconstruction:

    1. Tumor Removal (Lumpectomy):
      • The primary goal of the surgery is to remove the cancerous tumor along with a margin of healthy tissue to ensure all cancer cells are excised.
      • The extent of tissue removal depends on the size, location, and characteristics of the tumor.
    2. Breast Reconstruction:
      • After the tumor is removed, the breast is reshaped to maintain or restore its appearance. This reconstruction can involve several different techniques:
        • Tissue Rearrangement: The remaining breast tissue is rearranged to fill in the gap left by the tumor removal. This may involve mobilizing adjacent tissue within the breast to maintain a natural shape.
        • Breast Reduction or Lift: In some cases, the reconstruction may involve performing a breast reduction or lift on the affected breast. This is especially useful when a large amount of tissue needs to be removed, and it also helps achieve symmetry with the other breast.
        • Tissue Flaps: In more complex cases, tissue from another part of the body, such as the back (latissimus dorsi flap) or abdomen (TRAM or DIEP flap), can be used to reconstruct the breast.
    3. Symmetry Procedures:
      • To achieve symmetry between the treated breast and the other breast, the surgeon may perform additional procedures, such as a lift, reduction, or augmentation, on the non-affected breast.
    4. Cosmetic Considerations:
      • Oncoplastic surgery aims to minimize scarring and deformities, preserving the breast’s natural shape as much as possible.
      • The incisions are often strategically placed to be less noticeable, and the breast is reshaped to maintain a balanced and aesthetically pleasing appearance.
    5. Multidisciplinary Team:
      • The procedure typically involves collaboration between oncologic (cancer) surgeons and plastic surgeons. This team approach ensures that both the cancer treatment and cosmetic outcomes are optimized.
    6. Benefits:
      • Oncoplastic reconstruction allows for more extensive tumor removal without compromising the cosmetic appearance of the breast, which can be particularly important for women with larger tumors or smaller breasts.
      • It can reduce the need for more invasive procedures like mastectomy (complete removal of the breast) by allowing breast-conserving surgery with better cosmetic results.
 
  • Risks:
    • There might be a need for mastectomy if final pathology results show positive tumor margins (typically available after 7-10 days).
    • Risks include potential loss of the nipple, reduced nipple projection, or decreased nipple sensation.

 

Recovery and Outcomes:

  • Recovery may take longer than a standard lumpectomy due to the additional reconstructive work, but many patients find the outcomes rewarding in terms of both cancer control and body image.
  • The success of the procedure depends on the tumor’s location, the amount of tissue removed, and the patient’s overall health and expectations.
  • Radiation is generally still required after oncoplastic reconstruction.  Women may consult with a radiation oncologist prior to before their surgery to discuss their options

Dr. Jablonka is an amazing surgeon. During my consultation for DIEP I knew within minutes that he was the surgeon for me. Dr Jablonka is a phenomenal surgeon and he truly cares for his patients. I had a breast reconstruction with DIEP flap post prophylactic double mastectomy from 12 years prior. He removed the old implants using my own fat and placing them above the pectoral muscle not under like before. The look and the feel is so natural. I don’t have that constant tightness and hardness in my breasts. I feel and look normal again. He has been very kind throughout the process. He explained everything patiently every time.
