Nipple & Areolar Reconstruction

Nipple reconstruction is a surgical procedure often performed after breast reconstruction surgery, typically following a mastectomy. The goal is to recreate the appearance of a natural nipple and areola to enhance the overall look of the reconstructed breast. Here’s an overview of the process:

Timing

  • Delayed Reconstruction: Nipple reconstruction is usually done a few months after the initial breast reconstruction surgery to allow the breast tissue to settle and heal.
  • Immediate Reconstruction: In some cases, it can be done at the same time as the breast reconstruction.

 

Nipple Reconstruction Techniques

Several techniques can be used for nipple reconstruction, and the choice depends on the patient’s anatomy, the type of breast reconstruction, and personal preferences.

Local Flap Techniques

  • C-V Flap: Tissue from the reconstructed breast is used to create a new nipple. Small incisions are made to form a flap, which is then folded and sutured to shape a nipple.
  • Skate Flap: A more complex flap design where tissue is lifted and folded into a nipple shape.

Grafting

  • Skin Graft: Skin from another part of the body (often the inner thigh or abdomen) is used to create the nipple and areola. This technique can provide a good color match.

Tattooing

  • 3D Nipple Tattooing: A non-surgical option where a tattoo artist creates a three-dimensional appearance of a nipple and areola using pigment. This can be done alone or in conjunction with surgical techniques to enhance the appearance.

 

Areola Reconstruction

  • Tattooing: Pigment is used to create an areola around the newly formed nipple. This can be done in a range of colors to match the patient’s natural skin tone.
  • Skin Graft: Sometimes, a small skin graft can be used to create the areola, though this is less common.

 

Considerations

  • Symmetry: Efforts are made to match the size, shape, and position of the new nipple to the natural one or to create a symmetrical appearance on both breasts.
  • Sensation: Reconstructed nipples typically do not have the same sensation as natural ones, though some sensation may return over time.
  • Healing: Patients must follow post-operative care instructions to ensure proper healing and avoid complications such as infection or loss of the reconstructed nipple.

 

Risks

  • Infection: As with any surgery, there’s a risk of infection.
  • Loss of Nipple Projection: Over time, the reconstructed nipple may flatten or lose projection.
  • Asymmetry: Achieving perfect symmetry can be challenging.

 

Nipple reconstruction can significantly enhance the aesthetic outcome of breast reconstruction, contributing to a more natural and satisfying result for patients. It’s important to discuss all available options, techniques, and potential outcomes with a qualified plastic surgeon.

Dr Jablonka is a phenomenal surgeon and he truly cares for his patients. I had a breast reconstruction with DIEP flap post prophylactic double mastectomy from 12 years prior. He removed the old implants using my own fat and placing them above the pectoral muscle not under like before. The look and the feel is so natural. I don’t have that constant tightness and hardness in my breasts. I feel and look normal again. He has been very kind throughout the process. He explained everything patiently every time.
