Direct-to-Implant Breast Reconstruction

Direct-to-implant breast reconstruction is a surgical technique that involves placing a breast implant immediately after a mastectomy, in the same surgery. Unlike traditional methods that require a tissue expander to gradually stretch the skin and muscle before inserting the final implant, direct-to-implant reconstruction skips this step. The implant is positioned directly into the breast pocket created during the mastectomy, often supported by acellular dermal matrix (ADM) or a similar material to provide additional coverage and structure.

  • Immediate Results: Patients achieve breast reconstruction in a single surgical procedure, which can provide quicker emotional and aesthetic recovery.
  • Shorter Overall Treatment Time: By combining the mastectomy and reconstruction, patients avoid the extended process associated with tissue expanders, reducing the total time needed for reconstruction. This technique typically adds 30-60 minutes of operative time following a mastectomy.
  • Fewer Surgeries: Direct-to-implant reconstruction typically requires fewer surgeries compared to staged reconstruction with tissue expanders.
  • Natural Aesthetic Outcome: With the use of ADM, surgeons can create a natural breast shape, achieving a more immediate and aesthetically pleasing result.

 

Risks:

  • Implant Loss: There is a risk of implant loss due to infection, poor healing, or insufficient blood supply to the skin, particularly in patients who may need radiation therapy post-surgery.
  • Rippling: Visible rippling of the implant may be visible through the skin.
  • Capsular Contracture: This condition involves the hardening of the tissue around the implant, which can cause discomfort and affect the appearance of the breast.
  • Asymmetry: Achieving perfect symmetry between the reconstructed breast and the natural breast can be challenging, and additional surgeries may be required to improve the cosmetic outcome.
  • Limited Candidacy: Not all patients are suitable for direct-to-implant reconstruction; factors such as skin quality, breast size, and the need for radiation therapy may limit eligibility.
  • Complications with ADM: While ADM can enhance outcomes, it also introduces risks such as infection or seroma (fluid buildup), which can complicate the healing process.

Dr. Jablonka is an amazing surgeon. During my consultation for DIEP I knew within minutes that he was the surgeon for me. Dr Jablonka is a phenomenal surgeon and he truly cares for his patients. I had a breast reconstruction with DIEP flap post prophylactic double mastectomy from 12 years prior. He removed the old implants using my own fat and placing them above the pectoral muscle not under like before. The look and the feel is so natural. I don’t have that constant tightness and hardness in my breasts. I feel and look normal again. He has been very kind throughout the process. He explained everything patiently every time.
